New Patient Center

Active Life Chiropractic is accepting *new patient’s.

  • The first appointment typically takes about an hour and includes examination and treatments as appropriate.

  • The chiropractor will review your history and your pain complaints before recommending treatments which may include the additional therapies we offer.

  • Typically treatment is able to begin at the same appointment.

  • *Unfortunately we are not able to treat Medicare enrolled patient’s at this time.

Please arrive to your appointment 15 minutes early to fill out paperwork unless you filled out the paperwork online. (We send it out to you after you schedule). You will either use your own personal device or our check in computer to let us know of your pain complaints at each visit. We have a QR code at the office and then for subsequent visits you can visit the site up to an hour before your scheduled appointment to check in!

Insurance Questions

We are not in-network with any insurance company.

The receipts given after your appointments have information needed for you to send to your insurance company for reimbursement if you have chiropractic benefits. If you are unsure, call the number on the back of your insurance card and they can answer those questions for you!

We can also print a Superbill at the end of the year which will include all of your appointments on one sheet which is great for tax purposes.
