• If you are a NEW PATIENT: schedule Examination appointment.

  • **IF you are a new patient and schedule a chiropractic appointment but you are not a patient of record, we will not be able to accommodate you at your appointment and will have to be rescheduled as a new patient appointment.

  • **We can not treat Medicare enrolled Individuals as a patient currently at Active Life. Please call the office if you need a recommendation for an in-network practice.

• If you are an existing patient: schedule either a Chiropractic or Soft Tissue Appointment

• If you need an appointment within 24 hours, you must call the office to schedule as the system restricts ability to schedule same day appointments.

• At times, there may be more appointments open if you call the office that are not available on the online system.
